Enrollment Information

Residency must be verified to initiate the enrollment process:

1. Demographic information (address-home, work, cell phone numbers)
2. Birthdates for all family members
3. Copy of Birth Certificate for student being enrolled
4. Student’s social security card
5. Your employer’s name and address, if applicable
6. E-mail address (if you have one)
7. Copy of immunization records – Student’s immunization record must be current in accordance with Pennsylvania law
8. If your child is entering Kindergarten or 1st grade for the first time with our district, or coming from an out-of-state school district, your child will need a physical and dental exam. Physical and Dental forms can be downloaded from our website under “Student Services – Health Package”.
9. To enter Kindergarten your child must be five (5) years old on or before September 1 of the school year.

11. If applicable, please provide any existing transcripts, ER and IEP or GWR and GIEP for Special Education.

Please bring all completed forms to the school your child will be attending. Personnel at the building will give you additional information and will begin assigning your child a schedule. Please allow a minimum of three (3) working days from the date all records have been submitted, for the district to complete the registration process.
